In order to receive lunch, this form must be completed and submitted by the following Monday 8/31/20 by 8:00 am. All students will be charged for each meal unless your family qualifies for the free and reduced lunch/breakfast program. PLEASE ONLY USE THIS FORM for the days your student will NOT be on campus. If your student is on campus, there is no need to complete this form as their teacher will take lunch count.

NOTE: Failure to pick up the breakfast and/or lunch that you ordered for your student will result in a charge to your student's meal account.
