We would like to have an idea of how many plan to attend our outdoor Combined Worship Service on Sunday, April 18, at 11 a.m. The service will be held on the east side of the building.

Please complete this short online form with the number of people from your household who will be attending. We ask that each household complete its own form.

A household is a group of people who live together. Family members who do not reside in the same residence are not considered part of your household.

Protocols will be in place: Everyone must wear a mask. Bring your own chairs (which you must manage yourself). Follow the instructions of the volunteers for parking and where to sit. You will be asked to sign in for contact tracing purposes, and you will be provided with a bulletin.

The service will also be livestreamed on Facebook Live and our website homepage.
