Virtual classes (continue here) All boxes must be checked for this choice.

I understand that my home must have an internet connection.

I understand that my child will be expected to be logged into scheduled classes and be held accountable for attendance under Texas attendance law.

I understand that this selection is for one grading period. (9 weeks)

I understand that this selection will result in my child being prohibited from extra curricular activities for the grading period.

I understand that all grades will be issued by the teacher in the virtual classroom. There will be no exchange of books or paper during this virtual period.

Face to face classes (continue here) All boxes must be checked for this choice.

I understand that students must wear face coverings for the first nine weeks and potentially an extended period of time as the situation merits.

I understand that Crowell ISD has taken many mitigation methods but there is no perfect environment with the Corona Virus.

I understand that my child must practice social distancing.

I understand that my child's temperature will be screened daily upon entry to school.

I understand that part of the daily routines at school will involve handwashing.

Parent Safety Protocol (Please read and check all) *

I understand that parents and members of the public must not enter the school but will be asked to wait in the entry area.

I understand that members of the public including parents will be screened for temperature upon entry to the school.

I understand that at drop off and pick up parents must wait in their cars.

I understand that if my child develops a fever, I must come to the school or send a responsible adult in my place to pick up my child within 30 minutes.

I understand that Crowell ISD seek to maintain a safe environment for all. These procedures are for the safety of everyone.

I understand that if I have questions I may contact the teacher or principal by phone or email.
