Those testing for 1 Kyu and above must submit an essay - email to mwkfpromotion@gmail.com.  In the subject line of the e-mail, type "MWKF Promotion Exam (your AUSKF ID)". ESSAY MUST BE YOUR ORIGINAL WORK.
The regional permission form *

ONLY Kata - Testing Voucher (if applicable) *
If you have a voucher to only take the kata portion, please email to mwkfpromotion@gmail.com after submitting this application form.  In the subject line, type "MWKF Promotion Exam (your AUSKF ID)". Please also bring it with you to submit at the registration table.
Doctor Restriction Letter (if applicable) *
