Derwent Valley AAP area - Activities with ATOMS
Please read this carefully before completing the form.

This form is to allow us to allocate our free children's activity kits for our Summer half term project funded by Derwent Valley Area Action Partnership. To check  This project consists of a Cake Ninja Pasta Bake activity, a drink and dessert and a Science Ninja Vicious Volcanoes activity.  All instructions to carry out these activities with your children are included, however there are also opportunities for your children to attend Zoom workshops online on Wednesday 2nd June at 11am and 3pm . Links to these will be sent to the email address provided prior to the sessions or can be accessed via our facebook page in the run up to half term.

We will be delivering these kits to your home address during the last week of the half term, either directly or via one of our community partners in the area.

To qualify you must live in or your children must go to school in the Derwent Valley AAP area and there is certain information we need to feed back to our funders so we will need you to fill out this form fully in order to be considered for a free pack. This includes first and last names of children, dates of birth, postcode (we are asking for full addresses for delivery purposes only) and the school they currently attend. Please do let us know if your child has any additional needs.

Don't worry though all information is only used for the purposes of running this project and will be stored securely by ourselves to assist us with project delivery. Certain information will be shared securely with Durham County Council as part of the monitoring process of the funding we have received. If you have any questions about this or how we use your information please get in touch at atomsvonni@gmail.com. Included is the DCC data protection statement below:
