This form is for a waitlist for D6 Summer Connect enrichment sessions.  Currently there is:

No availability for K-2
Limited availability for 3-5
Availability for 6-8

If you are interested in having your child participate in a D6 summer connection session and wish to be placed on the waitlist, please fill out the form below. Please note that student placement is not a guarantee.  The purpose of this form is for you to convey your child's interest and for us to use the information to connect as many students to the summer sessions as possible. You will receive an email confirmation of the student's placement.

SUMMER CONNECT REGISTRATION IS OPEN UNTIL FRIDAY, JUNE 4, 2021.

This form will be for placing K-8 students in the enrichment sessions of Session 2 or Session 3 ONLY.

Session 2: July 12-July 22
Session 3: July 26-August 5

Transportation is available with bus stops and routes created after student placement. Some courses will incorporate excursions.

The locations for Summer Connect are Central Point Elementary (CPE), Hanby Middle School (HMS), Scenic Middle School (SMS), and Crater High School (CHS). See the course description for the specific location of each course.

IF YOU ARE ENROLLING MORE THAN ONE CHILD: This form must be completed separately for each child you enroll. At the end, you will submit your response for Child 1. Once you do, select "submit another response" and fill out the form for Child 2 and so on.
