Please complete this survey ONLY if yous student has items to collect from the locker room OR your child has a district Chromebook and will NOT be returning to a San Diego Unified School next year. Students will be given access to the locker room on the Collection & Distribution day to empty their locker. Chromebooks will be collected on the same day at the textbook station.

Complete esta encuesta SOLAMENTE si su estudiante tiene artículos para recoger en el vestuario O si su hijo tiene un Chromebook del distrito y NO regresará a una Escuela Unificada de San Diego el próximo año.
