MPS Employee COVID-19 Critical Service Support Interest Form

This form is to be filled out by MPS employees who are offering to provide support for critical services during the COVID-19 school closure period.

We will contact you as needs are identified. MPS staff should reach out to their supervisors for approval to engage in tasks that are beyond the scope of their assigned telework duties.

Please note- individuals in any high-risk group (older adult, chronic health condition, caretaker for a high-risk person, and others) should take precautions and stay home.
