LIABILITY RELEASE
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Laura Carson's dance class. I hereby agree to release to Laura Carson and hold Laura Carson harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation. If I am a minor, my parent and / or legal guardian has also read and agreed this document releasing Laura Carson from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. The participant has my permission to participate in Laura Carson's dance class. I warrant the information is complete and correct. I further release Laura Carson of all liabilities associated with my attendance at Laura Carson's dance class.

COVID-19
I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntary assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families. I release Laura Carson (Laura Carson Dance), its instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses.

PHOTO RELEASE
LAURA CARSON may include photos and/or videos of students, parents, and dance class activities on her website, social media accounts (Facebook, Instagram, Pinterest, YouTube) and/or printed marketing materials from time to time. I understand that these pictures will be accessible to anyone with internet access. We/I hereby give permission to LAURA CARSON to use photos on her website, social media accounts, in marketing materials, and/or electronic forms of communication.
