It is really important at Harrow Way Community School that we have up to date information for our student medical files.

Please can you take the time to fill out the form and inform me of any medical changes that have occurred throughout the year. For example: onset or change to, conditions such as asthma or allergies, heart conditions, bladder conditions, need for regular medication, prone to fainting, eye or ear conditions.
