This form is used to collect required COVID test and vaccination information for children and adults.
When do I use this form to certify COVID tests results?
- First Week of School: All staff and students + any parents/approved caregivers entering the building for drop off & pick up will test for COVID the first week of school prior to orientation (test should be taken day before or morning of).
- Prior to Parent Participation Day: All parent participants must test the day before or morning of their participation day in the classroom.
- After Major Breaks: All staff and students + any parents/approved caregivers entering the building for drop off & pick up will test and provide certification of negative results after major breaks such as Thanksgiving, Winter Break, MLK, Presidents Day, Spring Break, and Memorial Day Weekend.
- After Travel: For travel to any US or International destination that is designated at a high COVID community level, take a COVID test after travel and prior to returning to school. At this time we do not require you to wait a designated amount of time after travel in the US or internationally before testing, but this may change depending on any new developments. If the decision is to not test, then individuals will stay home and self-quarantine for 5 days after travel.
Should you have COVID symptoms or have been exposed to someone who is COVID positive, please immediately call the office for guidance. COVID test documentation in these situations will need to be sent directly to office@silverspringdayschool.org. Please do not use this form in these situations.
Please note: we accept at-home or PCR tests. If using an at-home test be sure to use a currently valid test (some tests have updated expiration dates). Also, please be sure that any at-home test used for a child is authorized for 2+ and EUA authorized (such as iHealth or BinaxNOW).
Are you a parent participant, staff, sub, or volunteer? At the end of the form, you will be asked to certify that you have completed your COVID primary series vaccination. Thank you for doing this each time you complete the form.
- The COVID primary series vaccination is required for all staff (including subs), parents who are participating in the classroom, and any volunteers. We highly recommend boosters when available and eligible.
- SSDS strongly recommends that children be vaccinated now that the vaccine for 5 years and under is available. Currently COVID vaccination is not required for children.
