EVC Dual Enrollment Request Form
Thank you for your interest in Dual Enrollment at EVC this Summer 2023! Please submit this form to confirm your interest.

There are 3 sections to complete this form:
  1. Student Information - You will need your EVC ID#, if you do not have one yet, please stop and apply to EVC at: https://bit.ly/apply2evc (You will receive your 7-digit EVC ID# within 3 business days)
  2. Course Selection - Select your first and second choice
  3. Required Documents - CCAP Form (or Special Admit form for some students) and HS Transcripts (for Math Academy students)
Submitting this form does NOT automatically enroll you into the course of your choice. You must be approved by your high school counselor to take any courses at EVC and will receive communications from the Dual Enrollment team about next steps.
