This form is used to pre-approve Certified Public Accountants (CPAs) and Certified Public Accounting Firms (CPA Firms) interested in being listed in Arizona Film & Digital Media's Production Directory to provide Assurance services to Production Companies seeking to file Post-Qualifying applications with the Arizona Commerce Authority (ACA) for the Arizona Motion Picture Production Program (MPPP) tax-credit.

**PLEASE NOTE: A Production Company will separately file a CPA Approval Form with the names of the CPA Firm and CPA who they desire to use, via the MPPP Application Portal, for ACA verification to ensure license and assurance practice remains active at the time of service to the Production Company.
