2020 Summer Strength & Conditioning
WORKOUT DATES:  Week of June 8-11 through week of July 20-23

NOTE:  No workouts will be held June 29 - July 10

LOCATION:  FA Weight Room/Field

COST:  $75 per person

TIMES:
      HIGH SCHOOL (9th - 12th) ~ 9:00 am to 10:30 am
     JUNIOR HIGH (6th - 8th) ~ 10:45 am to 12:15 pm

REGISTRATION FORM & PAYMENT DUE DATE:  Prior to beginning the program; the registration fee will be $75 no matter what date athlete begins program.

All student-athletes must be enrolled, or in the enrollment process, at FA for the 2020-2021 school year.  Payment will be charged through your FACTS account.

General Statement of Program Objectives and Procedures:  I understand that this physical fitness program includes exercises to build the cardiorespiratory system (heart and lungs), the musculoskeletal system (muscle endurance and strength, and flexibility), and to improve body composition (decrease of body fat in individuals needing to lose fat, with an increase in weight of muscle and bone). Exercise may include aerobic activities, callisthenic exercises, and weight lifting to improve muscular strength and endurance and flexibility exercises to improve joint range of motion. *
Potential Risks:  I understand that the reaction of the heart, lung, and blood vessel system to exercise cannot always be predicted with accuracy. I know there is a risk of certain abnormal changes occurring during or following exercise which may include abnormalities of blood pressure or heart attacks. Use of the weight lifting equipment and/or engaging in heavy body calisthenics may lead to musculoskeletal strains, pain and injury if adequate warm-up, gradual progression, and safety procedures are not followed. I understand that Fellowship Academy coaches shall not be liable for any damages arising from personal injuries sustained by the athlete while and during the training program. The athlete using the exercising equipment during the training program does so at his/her own risk. The athlete assumes full responsibility for any injuries or damages which may occur during the training. *
Potential Benefits:  I understand that a program of regular exercise for the heart, lungs, muscles and joints, has many benefits associated with it. These may include a decrease in body fat, improvement in blood fats and blood pressure, improvement in physiological function, and decrease in heart disease.   *
I understand and warrant, release and agree that my child is in good physical condition and has no disability, impairment or ailment preventing him/her from engaging in active or passive exercise that will be detrimental to heart, safety, or comfort, or physical condition. *
By clicking "I agree" I give my permission for my child to participate in the Fellowship Academy Summer Workout Program and acknowledge that the fee of $75 will be charged to my FACTS account.  This authorization shall forever waive, release and resolve Fellowship Academy, the staff and coaches from any and all liability from injury and or illness incurred by the player, parent, sibling or spectator. I give the staff permission to act on my behalf, according to their best judgment, in any emergency. I also certify that my child has no physical problems or disabilities which would impede from participating in the Summer Workout Strength & Conditioning Program. *
