- You may update this form at anytime before May 15th. DEADLINE FOR ALL SUBMISSIONS/UPDATES IS MAY 15, 2021. Changes/Registrations made after the 15th, may not be refected in printed materials.
- Your registration is not complete until payment has been recieved. A payment link is provided at the end of this form.
- If you have a full team, list names, phone numbers, and email addesses of each member at the time of registration.
-  If your team has less than 4 members, list the members you have and opt for either *I WILL FILL MY OWN TEAM or *FILL MY TEAM FOR ME for the remaining team member spots.
- You can update this form at any time to finish providing your team members information.
- If you are signing up as a free agent, please list FREE AGENT as your team name and select *FILL MY TEAM FOR ME for the remaining team member slots.
