I, the undersigned, acknowledge that participation in Box Nutrition and Performance's testing, exercise and fitness programmes exposes me to inherent risks, including but not limited to equipment malfunction, accidental injuries, and medical disorders.

Waiver and Release:

In consideration of the acceptance of my participation, I hereby waive and release any claims or rights against Box Nutrition & Performance, its instructors, employees, and representatives from all liabilities, actions, claims, damages, costs, or expenses which I may have arising out of any injury, accidental or otherwise, during my participation in any test, activity, class, or programme at Box Nutrition & Performance.

Assumption of Risk:

I assume all risks, both known and unknown, and full responsibility for my participation and use of Box Nutrition & Performance's facilities and hereby release and hold harmless Box Nutrition & Performance, its instructors, employees, and representatives, with respect to any and all injury or illness, disability, or loss or damage to person or property, to the fullest extent of the law.

Personal Responsibility:

I acknowledge that it is my responsibility to consult with a healthcare provider prior to and regarding my participation in any test or exercise programme at Box Nutrition & Performance. I represent and warrant that I have no medical condition that would prevent my participation in physical activities.
