2024-2025 Confirmation Preparation Step 1
Everyone who wishes to receive the sacrament of Confirmation in the Concord-Carlisle Catholic Collaborative, planned for the spring of 2025, should complete this online form to request preparation for and reception of the sacrament of Confirmation. Receiving sacraments is not something done "automatically" because one is enrolled in Faith Formation or reached a certain age or grade. One receives a sacrament because they are "suitably prepared", and that involves Faith Formation to prepare to receive the Sacrament of Confirmation.

This online form is not a substitute for the Faith Formation registration! Click here to go to our website to register online for Faith Formation.

This online form should be used by teens who are registered for, and participating in, Faith Formation in the Concord-Carlisle Catholic Collaborative.

Confirmation Preparation Step 1 includes completing this online form and then meeting with Tony Carbrello, Coordinator of Youth Ministry. Tony looks forward to reviewing these responses, having a conversation about your faith journey, and your speak with you about the preparation process for receiving the sacrament of Confirmation. Please sign up to meet with Tony with the link at the conclusion of this online form.

This is an information form, not a quiz, test or exam! There are no "right" answers, just honest ones. Tony and his team are here to accompany teens on their journey of faith. In order to accompany, we need to get a sense of where each teen is at, now, on the unique journey of faith...

Worship and Sacraments:
Tony's personal reflections on the Mass
When I was young, I thought Mass was boring! I complained about going the few times we went, which is probably why my family didn't attend Mass much. Really, I only started going to Mass regularly when I received my driver's license, because I realized that if I was going to take Confirmation seriously, I should get serious about whether or not I wanted to be a Catholic.

What I have learned is that the Mass is rich in meaning and the more I learn, the more meaningful the Mass becomes. But that doesn't mean that you (or I) have to know everything about the Mass now.

It's okay if you don't like going to Mass or don't attend Mass much - we can talk through that during our conversation.
What is your experience of attending Mass (in the past few years)? Check all that apply. *
Required
Do you like to attend Mass? Why or why not? Is there anything about the Mass that you wish were different? What improvements would you recommend we make to how we do Mass at Holy Family and St. Irene?
When was the last time you experienced the sacrament of Reconciliation? (sometimes called Confession or Penance) (if you don't remember, just say so... it may have been when you were in second grade and/or whenever you did it for the first time) *
Longer Answers - Going Deeper (for teens and an adult)
The following longer-answer questions may be best prepared in a separate document, then cut-and-paste your response into this form. Candidates should answer one question, and a parent should answer one question. The questions are below. There are no right or wrong answers. Your responses will give Tony and his team a better sense of your family's intentions for the sacrament as well as respecting where the teen candidate is on his/her faith journey.
Candidate Question - please answer one of these two questions:
1. What type of Christian man or woman do you envision being in the future? OR
2. What steps do you need to take to get closer to God?
*
Parent / Guardian Question - please answer one of these two questions:
1. How have you shared with your teen the importance of the sacrament of Confirmation? OR
2. What is the hope or vision you have for your teen after he or she receives this sacrament?
*

Long answer questions are shared from the book Rebuilding Confirmation by Christopher Wesley.
Some Final Points, for Now:
Just a couple of final questions, for now, and then you need to choose submit and schedule a time to speak with Tony!
Questions that I (the candidate) have about the Catholic faith are... (these may be addressed in conversation with Tony)
If someone were to look at your life (from the outside), how much evidence would they see that you are a Catholic Christian? (things, actions, speech, etc.)
not that much
quite a bit
Clear selection
Why? or Why not? (be honest) What evidence would someone see?
What gifts has God given you? What are you good at, seemingly better than others? What comes more easily to you than others? What have others said that you are good at?
