2023 Confirmation Step 1
Everyone who wishes to receive the sacrament of Confirmation in the Concord Carlisle Catholic Collaborative, planned for the spring of 2024, should complete this online form to request preparation for and reception of the sacrament of Confirmation. Receiving sacraments is not something done "automatically" because one are enrolled in Faith Formation, though "suitable preparation" is expected to receive a sacrament, and that involves Faith Formation.

This online form is not a substitute for Faith Formation registration! Click here to register online for Faith Formation. This should be used only by teens who are registered for, and participating in, High School Faith Formation in the Concord-Carlisle Catholic Collaborative.

Step 1 includes completing this online form and then signing up to meet with Tony Carbrello, Coordinator of Youth Ministry. Tony looks forward to reviewing these responses with you and having a conversation about your faith journey and your receiving the sacrament of Confirmation - you will receive a link to sign up for a time at the conclusion of this online form.

This is an information form, not a quiz, test or exam! There are no "right" answers, just honest ones. Tony and his team are here to accompany teens on their journey of faith. In order to accompany, we need to know where we each are on that unique journey...

Tony's reflections on the Mass: when I was 15-16, I thought Mass was boring! Really, I only started going regularly when I received my driver's license because I thought, if I was going to take Confirmation seriously, I should get serious about whether or not I wanted to be a Catholic. The Mass is rich in meaning and the more I learn, the more meaningful the Mass becomes. But that doesn't mean that you (or I) have to know everything about the Mass now. It's okay if you don't like going to Mass - we can talk through that during our conversation.
