Please fill out this form to request memorial plaques (pai weis) for someone, living or deceased, you wish to dedicate merit to. We ask that people who request to put up a plaque to attend our regular BBM Online practices.

無論為生者還是亡者祈福, 請您填妥以下牌位申請表格。本寺要求申請牌位者, 參加我們線上例行的共修活動。
Guidelines:
- Two plaques per person. If there are exceptional circumstances, you can request for more than two plaques. (e.g. recent death or sickness)
- Plaques will be put up for 49 days.

牌位登記須知:
1)一人僅限申請兩個牌位。除非個案情況特殊, 例如往生者或病人, 本寺將會開緣處理。
2)每期牌位將供奉49天。
