Please submit the information in the form below to confirm your interest in participating in a usability evaluation for a newly developed medical product. Please be honest! We are currently looking for individuals and parents with children 2-13 years of age who have been diagnosed COVID-19 positive within the last 7 days or are exhibiting symptoms of COVID-19, the flu, or a cold to participate! Being exposed or having COVID, flu, or cold like symptoms will not disqualify you so please be honest in the screener. This study will be conducted at our facility in San Jose, CA in person or if you choose, at your home. For everyone's safety, COVID-19 precautions and procedures will be strictly followed. We are currently looking for adult/child teams, teens, and adults.
Usability Evaluation Details:
Dates: Open through March 2022
Length of Session: Approximately 1 hour
Location: 14120 Beach Blvd #101, Westminster, CA 92683
Incentives: $200 for Adults and Adolescents or $250 for Adult/ Child Teams
If you are under 18 your parent or legal guardian will be required to be present in person to complete paperwork agreeing to your participation in the study.
