CORI SUBMITTED?

Have you submitted a CORI form for the Duxbury Public Schools within the past three years? A CORI is valid for three years. If you know your CORI has expired or you do not have one on file for Duxbury Public Schools, please print a copy with link provided below. Bring the completed CORI form along with a copy of a valid photo ID with you to the Chandler School Main Office to be processed. 

When do you need your volunteer status checked?

Please select ASAP if you are signed up to volunteer within the next two weeks or Not Sure if you would like to volunteer at some point during the school year. Your CORI status will be checked and you will be emailed if you need to submit a CORI form to be eligible to volunteer. 

VOLUNTEER ORIENTATION COMPLETED?

By submitting this form, you agree that you have completed Volunteer Orientation for Duxbury Public Schools. Volunteer Orientation must be completed ONCE per lifetime for Duxbury Public Schools. If you have not completed orientation or are not sure, please use this form (Chandler Volunteer Orientation Form) to request the Orientation packet from Chandler School. Please save the email of your submitted Volunteer Registration Form to edit your reply to YES once you have completed orientation. You are NOT eligible to volunteer until you have completed orientation.

By accepting a volunteer assignment for the Duxbury Public Schools, you agree to respect the confidential nature of your assignment. You understand that as a school volunteer, you may become aware of confidential information about specific students. You will remember that discussing this student information is unethical. You understand and agree that you will not disclose such confidential information about students to anyone other than authorized school department employees. Failure to follow expectations of student confidentiality may result in discontinuation of my volunteer duties.

If you are not sure if you have a CORI on file, please submit the Volunteer Registration Form and the office will check if you have a current CORI on file.
