In order to TERMINATE your membership with APEX School of Movement, San Diego please fill out the form below. We do offer the option to suspend your membership if you are merely unable to attend classes temporarily.

A suspension will allow you to freeze your membership for a term of your choosing. This freezes your classes and your payments until the suspension ends. A suspension also allows you to retain any discounts you have on your membership. Please see the ACCOUNT SUSPENSION FORM or contact us for more information.

Please notify us ONE MONTH (30 days) in advance of your next billing date in order to terminate your membership and cease autopays.

I certify that I have authority over the account(s) for the people listed below and I would like to terminate their memberships with APEX School of Movement, San Diego. I understand that, upon termination, all seasonal or special discounts on these memberships will be lost and that I will be required to pay a registration fee if I enroll again.

PLEASE NOTE: This form is required and there are no exceptions. Once you complete the form, you will receive a copy of the form via email. If you do not receive the email, please either resubmit the form or contact APEX San Diego. The email may end up in your junk inbox.
