By completing this online form, I do hereby consent to voluntarily participate as a member of the Midcoast Umpire Association. I further acknowledge the following:

• I understand the fitness standards involved with being an umpire, and attest that to my knowledge, I am physically capable of performing my duties as an umpire. Further, I release the Association and its officers from any liability or civil action concerning my personal health matters as they relate to my officiating on behalf of the Association as an independent contractor.

• I understand that I am an independent contractor, and that my association membership is dependent upon the requirements set forth in the Midcoast Umpire Association of Maine By-Laws.

• I understand that it is my responsibility to read and understand all applicable rules and mechanics documents, including NFHS rules, Busline League rules and the Maine Principals Association Bulletin, and further confirm that it is my responsibility to enforce all rules as formally interpreted.

• I acknowledge that I must attend all cited mandatory clinics and meetings, as stated in our Association By-Laws and as published via the Association’s annual calendar of events.

• I acknowledge that I have not committed a felony, and that I am subject to a background check, per Maine Principals Association and Maine Department of Education standards.
