Please note that completing this form does not guarantee that you will be assuming the role of "Sponsor", as Sponsors must be in communion with the Catholic Church in the way they choose to lead their lives. This is due to the duty and responsibility that a Sponsor carries in this ministry i.e. to be a role model to someone seeking God.

Note on PDPA:

By completing this application form, you would have given us consent to collect, use and disclose your personal data for the purpose of you receiving religious instructions through our RCIA classes. Your data will then be recorded in our registry and the same information may be sent to the Archdiocese for archiving.
2. Information in our registry is kept for future reference and may stay with us for a lifetime.
3. In that regard, you may want to ensure that the information provided is accurate and correct for your own benefit and convenience.
4. We may use your data to contact you on matters related to your RCIA.
5. All other provisions that we have taken to comply with PDPA will be applied to your personal data during the time you are with us.
