THIS APPLICATION FOR HIGH SCHOOL STUDENT TRANSCRIPTS IS ONLY FOR THE USE OF AND MUST BE SUBMITTED BY THE FORMER STUDENT AND MAY NOT BE USED BY THIRD PARTY REQUESTORS.

2. If you graduated in or before 2012 please go to HTTP://CPS.EDU/STUDENTRECORDS to request your records from the Office of Former Student Records.
3. Current GWHS students, please reach out to a counselor or college/career coach for your request.
