This is an account where an individual set ups for his or her single/one (1) family member who is above 55-years old (Adult) or below 18-years old (Child) who is not able to manage their screening and also have access to the application.

Here, the main account user (who may or may not have their own individual user account) will be able to manage and control all the functions of his dependent’s Patient Module and all the functions provided. This account is only for a single dependent, and if there is more than one (1) dependent, multiple dependent accounts will have to be opened. In this type of account, registration will include the requirement of a pre-signed consent form provided by the dependent (and clearly establishing the relationship with the individual main account user) with the authority given to the individual (main account user) to manage the application and access of the screening data that is stored based on the tests taken.

Once you have submitted this form, we will notify you of the next steps of the registration including opening of the account and confirmation of the screening booking. We will also notify you on the payment.
