Please fill out the following information for summer sports skill camps. Camps will be Tuesdays - Thursdays from 12:30-3:30 and are all co-ed camps. The cost is $50 per camper and includes a T-shirt. All campers should provide their own snacks, drinks, and lunches if needed due to allergies. YOU MUST GO TO GOFAN to PAY for the camps. SOCCER CAMP IS FULL-

I further understand that the athletic department does not carry insurance on students attending camps and by signing below I am affirming that I have medical coverage for my child.
