Promise Resident Application Information Request for Homeschooled or Non-Mason County-Based High School Graduates

All Mason County Resident Graduates who apply to the Mason County Promise as HomeSchooled applicants or applicants who do not attend a high school based in Mason County as of 2023 must complete this form and provide supplemental information in order for the Promise Zone to process their application.

ALL APPLICANTS: Proof of residency will be required prior to scholarship approval. Proof can be provided digitally to info@masoncountypromise.org or in person at the Promise Zone office. I will provide proof of the following to the Mason County Promise Zone Director for the purposes of verification of residency: *

Rent receipts for at least one year of residency in Mason County (including the student's graduation year)

Homestead tax receipt for at least one year of residency in Mason County (including the student's graduation year).

Residential utility (water bill, power bill, gas bill) receipts that demonstrate at least one year of residency in Mason County (including the student's graduation year)

HOMESCHOOLED RESIDENT APPLICANTS ONLY: Proof of homeschooled registration will be required prior to scholarship approval. Proof can be provided digitally to info@masoncountypromise.org or in person at the Promise Zone office. I will provide the following to the Mason County Promise Zone Director:

A copy of the letter received verifying my homeschool's registration with the state of Michigan

A copy of verification from the West Shore ESD (or another Mason County-based school district) that the student/student's family is registered for educational services or participates in public-school-based educational programming

RESIDENT APPLICANTS WHO ATTEND A NON-MASON COUNTY-BASED HIGH SCHOOL ONLY: Please provide a copy of your final high school transcript to the Mason County Promise Director. Transcripts should be mailed or emailed directly from your school to info@masoncountypromise.org or in person at the Promise Zone office, 116 S. Rath Ave., Ludington, MI 49431. Please list the school from which your transcript will be sent, below:
