NLAE Library Permission Form
All parents and students must agree and understand the NLAE library guidelines in order for students to check out books. This form MUST be completed every school year.

Your child will not check out NLAE library books until this form is filled out.

Please fill out one form per child that attends NLAE.

Must fill out even if you do not want your child to check out books at NLAE library.

LIBRARY RULES

▪ Kindergarten & First grade students are allowed to check out 1 book at a time.
  2nd grade - 8th grade students are allowed to check out 2 books at a time.
  Borrow time is 2 weeks.

▪ Books must be checked out and returned on your
child’s specified library day.

▪ If a book is not returned and limit is reached, no additional books can
be checked out.

▪ Fines will be charged for lost or damaged books. $8 Soft cover books - $20 Hard cover books

▪ Lost books must be paid for by parents/guardians before report cards are issued
