2023-24 Marching Band Uniform Inquiry, Glove, and Shoe Order Form
Please fill in your student's information so they can be best matched with a uniform.  During band camp, all participants will try on uniforms. Sizes will be kept confidential. Thank you!

The first part of the form is for marching shoes and/or gloves (they need several pairs of gloves for the season), please answer the questions and include the total amount of money due (if applicable).  Please send a separate check made out to Cosby Band Boosters on the first day of band camp and write "Shoes/Gloves" on the memo line, along with the student's name.

Note: Sizes for shoes or gloves will be verified during band camp.  We will have shoes & gloves available to try on and sizes will be recorded at that time. Percussionists and Guard do not use these gloves, but please still complete the questions so we have a record for each student.
