RELEASE AND WAIVER OF LIABILITY

I (Parent/Guardian or Senior Student) acknowledge that the LHS Student Code of Conduct will be enforced at the event and understand that my parent/guardian will be called and may be asked to transport me from the event at Legacy 925 if I (Senior Student) engage in behavior prohibited in the LHS handbook.

I (Parent/Guardian or Senior Student) understand and agree that I can participate in all events including Go-Karts, Trampolines, Dodge Ball, Warrior Race, Rock Climbing Wall, Jousting, Foam Pit, Axe Throwing, Laser Tag, Corn Hole, Curling, Escape Room, Arcade, Photo Booth and Casino Games.

NO OUTSIDE ITEMS:

I (Parent/Guardian or Senior Student) understand that I will NOT be allowed to bring ANY outside items with the exception of my cell phone, charger, or any necessary personal items. I may not bring a purse or bag as one will be provided for me. My car keys and any personal items will be placed in a bag which will be held by a parent and returned back to me upon return to Linden Schools.

CLOTHING:

I (Parent/Guardian or Senior Student) understand that I should wear comfy clothes and shoes. Linden Project Graduation will provide each student with a bag in which they may place any extra clothes/shoes or any personal items in.

TRANSPORTATION:

1. I (Parent/Guardian or Senior Student) understand that I must use the transportation provided by the Linden Project Graduation Committee to and from Legacy 925 to be granted permission to attend the event unless otherwise arranged with the Linden Project Graduation Committee.

2. I (Parent/Guardian or Senior Student) am aware that I will be leaving Linden Schools at 8:00pm June 2nd and returning to Linden Schools at approximately 4:00am June 3rd.

3. I (Parent/Guardian or Senior Student) understand that I must arrive at Linden High School and check in no later than 7:40pm. I understand the bus will be leaving promptly at 8:00pm.

4. I (Parent/Guardian or Senior Student) understand if I miss the bus, I will NOT be able to attend the event. 

5. I (Parent/Guardian or Senior Student) am aware that I will not be allowed to leave at any time between arrival at event and 3:00am when event ends.

6. I (Parent/Guardian or Senior Student) am aware that if I win a raffle item I am responsible for transporting my item from LHS to my destination.

This is a drug and alcohol free event put on by parents for Graduating Seniors to celebrate with friends in a safe environment. UNDER NO CIRCUMSTANCES will any senior who is under the influence and/or is in possession of drugs, alcohol or vapes be admitted to the party. The Linden Project Graduation Committee will follow these procedures in the event that a senior is found to be under the influence, and/or in possession of drugs and/or alcohol:

1. The parent/guardian of the senior will be contacted.
2. The seniors will be held in an area separate from the party.
3. Under no circumstances may any senior who has used or is in possession of drugs and/or alcohol be allowed to leave the party except in the custody of their parent/guardian, or the Oxford Police Department.

PHOTOGRAPH/VIDEO RELEASE:

I (Parent/Guardian or Senior Student) grant and convey to Linden Project Graduation all rights, titles, and interests in any and all photographs, images, video, or audio recordings of me or my likeness made by Linden Project Graduation in connection with participation in the event.

MEDICAL TREATMENT:

We (Parent/Guardian or Senior Student) release and discharge Linden Project Graduation from any claim whatsoever which arises or may arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during participation. In the event of an emergency requiring medical attention, I authorize the chaperones to seek any necessary medical treatment required for the student. I will accept responsibility for all expenses resulting from treatment if these expenses are not covered by my insurance.

EMERGENCY CONTACT:

The emergency contact provided understands they need to be available to be reached on June 2nd between 7:30 pm and 5:00 am on June 3rd. 

EMERGENCY PHONE NUMBER for SENIOR AND PARENT/GUARDIAN:

If you should need to get in touch with your senior or if your senior has any issues while attending the event we will have an emergency station setup for any incoming and outgoing calls. The emergency contact number will be provided to both parents and participants.

I (Parent/Guardian or Senior Student) hereby recognize that participation in the Linden Project Graduation Senior All Night Party at is completely voluntary and I (Parent/Guardian or Senior Student) assume all risks and dangers, known or unknown, associated with participation in the event. 

I (Parent/Guardian or Senior Student) understand these risks and dangers subject me to the possibility of physical injury (which could be minimal, serious, and/or result in death) and loss or damage to personal property. 

I (Parent/Guardian or Senior Student) hereby release and hold harmless Linden Community Schools, the Linden Project Graduation Committee (it's officers, directors, and any parents/volunteers), Legacy 925 (including officers, directors, employees, and all businesses connected with Legacy 925), and Las Vegas Knights from any claim, demand, loss, liability, damages, attorney fees, medical and any and all costs that I may incur due to injury, loss or damage of personal property, that I may sustain as a result of my participation in the event, including those sustained on the premises where the event is conducted and while I am traveling to/from such premises, regardless of the location or mode of transportation.

BY COMPLETING THE INFORMATION BELOW, I (Parent/Guardian or Senior Student) ACKNOWLEDGE AND REPRESENT THAT I have read the entire Release and Waiver Of Liability as stated above. I understand it and sign it voluntarily as my own free will; I am at least eighteen (18) years of age and fully competent; and I execute the Agreement for full, adequate and complete consideration fully intending to be bound by same.
