Completing this form is the FIRST STEP for registering your Pre-K/Head Start child. All parents/guardians will have to come in person to Districtwide Registration, July 21 & 22, at CISD Central Administration Building.
PLEASE NOTE: The final page of this form lists the documents that will be required to complete registration. Please be sure to bring ALL this paperwork to Districtwide Registration in order to fully register your child.
