Chalk-A-Doodle Festival Entry Form

Theme: "What the World Needs Now..." (love, gratitude, unity, kindness...)
Location: Glendale Marketplace located at 106 S Brand Blvd, Glendale, CA 91204

Please read all of the information, then fill out the questions below:

1. You must register below to participate as an artist. You may participate as a chalk artist or a doodle artist (or both) using the theme as a prompt.

2. For chalk artists: You will receive a portion of sidewalk for your mural approximately 5x5.  Keeping with the theme, we would appreciate you to follow these simple guidelines: 1) no nudity 2) no profane language 3) no political statements. Please submit a title for your work if you have one. In addition, if you have any promotional material for your art please provide.

3. For doodle artists: No need to be a professional artist - Create anything in the theme on a preferably 6x8" paper (no larger than 8'1/2 x 11) sheet using any medium —drawings, watercolors, or mixed media. Submit your doodles onsite at participating stores starting July 1. Your doodles will be showcased at the 2-day chalk-a-doodle festival art exhibition! Free free to sign your doodles. 

4. Everyone is encouraged to participate. Multiple doodles from one family is encouraged. This is a family and kid-friendly festival, please kept that in mind as you design for the theme.

Chalk artists must be 15 years of age or older.
Doodle artists can be any age.

Participating chalk artists will begin their murals on Saturday and complete their murals to last for a full two-day experience. All artists are treated to free chalk and fellowship!

I understand that all images must be suitable for public viewing.

The undersigned artist (guardian if under 18) grants non-exclusive license to Pinetree, Chalk-A Doodle Festival, and AMARA ONONI Event Design to re-use, publish, and re-publish photography of the artist (including minors) in any medium. The undersigned artist further grants Pinetree, Chalk-A Doodle Festival, and AMARA ONONI Event Design use of the artist's name (including minors) and likeness in any media unless the artist requests otherwise in writing.
