Registration form for our first service this Sunday, March 7th.
LOCATION: 477 Kingston Rd, Pickering (The Bridge - Church for All Nations)

CAPACITY: 80 people
*Registrants are guaranteed a spot on a first come first served basis. You MUST REGISTER EACH WEEK. This form CAN ONLY REGISTER ONE PERSON AT A TIME. EACH FAMILY MEMBER MUST BE REGISTERED INDIVIDUALLY. This form will close once our capacity for this service is full.*
Our weekly in-person Sunday services have resumed. Pre-registration is required to attend the service due to government regulations and limited seating. Remaining spots will be for walk-in attendees.
We'll send an email by Saturday if there are any changes due to government updates or other reasons.

Our online services will continue to be posted on our YouTube channel after the service. We love and miss those who choose not to attend in person at this time!

- I have completed Ontario's self-assessment:

- I am not sick or displaying symptoms of COVID-19: fever, breathing difficulties, breathlessness, cough, sore throat, fatigue or tiredness
- If I have been in contact with anyone infected with COVID-19 in the last 14 days, I have been tested and my result was negative and I continue to self-monitor for symptoms (if you have not received your results yet, please abstain from attending)
