KA Water Bottle Order Form
Use this form to place your order for the KAIS Yearbook water bottle sale. These water bottles are double-insulated, high quality, "Hydro Flask-style" water bottles that will last you season after season. 

NO PAYMENT CAN BE MADE ONLINE!! Your order is not guaranteed until payment is received. Please send cash or check (made payable to "KA Activity Fund - Yearbook") to Mr. Capanna in room 206 at the Intermediate School. Payment must be received no later than Friday October 7th, or your order will be cancelled. 

Please submit a new response for each additional water bottle you'd like to order. You will be given a link to submit another response after completing the form. You may submit one payment for all of your water bottles.
