Change in Learning Environment Request

This form should be completed when a parent/student would like to request to change from At-Home Learning to In-Person Learning.

As a reminder, the students who elect to participate in At-Home Learning must commit to doing so for the duration of the grading period.  Any request to transition from At-Home to In-Person learning may take place at the end of the 6 weeks grading period. Requests must be completed based on the following dates:

End of 1st Six  Weeks--Request change by September 17, 2020

End of 2nd Six Weeks--Request change by October 26, 2020

End of 3rd Six Weeks--Request change by December 14, 2020

End of 4th Six Weeks--Request change by February 15, 2021

End of 5th Six Weeks--Request change by April 5, 2021
