Please fill out this form if your in-person student has an absence. One of these forms needs to be completed for each day of absence. This form just tracks your student engagement, make-up work will still need to be completed for their academic grade.

Qualifying educational activities include: Classroom Assignments, Reading, Educational TV programs (Animal Planet, National Geographic, etc), Educational Computer programs (Prodigy, MobyMax, typing, etc), Art, Healthy Living Activities, Skill Building Activities. If you think something is educational please list it below and the homeroom teacher can decide.
