CEIS MAKATI ONLINE REGISTRATION AND ENROLLMENT FORM (OREF) FOR S.Y. 2023 - 2024
•ONLINE REGISTRATION for NEW STUDENTS shall start on March 4, 2022.
Notification to PROCEED for ENROLLMENT shall be sent starting June 1, 2022.

•ONLINE REGISTRATION AND ENROLLMENT for CONTINUING/OLD STUDENTS shall start on July 12, 2022.

PROCEDURE FOR ONLINE REGISTRATION AND ENROLLMENT
New Students
1. Applicant fills-up the online enrollment form accessed in the following links:

This is also posted in the CEIS FB Page.
2. Applicant uploads the admission requirement listed below. The documents should be
in jpg or pdf form.
• Form 138 (Card)
• ESC/Voucher Certificate
• School Certification e.g. Good Moral/Completion/Ranking for Rank 1-3 with 100
and above JHS Completers for Incoming Grade 11 applying for Scholarship
• PSA Birth Certificate
• DPA Form downloadable thru:

3. Enrollment staff notifies the applicant if his/her application is accepted, and issues applicant/
student number
4. Applicant pays the application and registration fee and tuition through the electronic
payment system by going to the Online Payment System at CEIS website and click the “pay now” button and follow the procedures.
5. Enrollment staff sends a copy of the certificate of registration to the student through email within 48 hours.
6. Student prints the certificate of registration as proof of enrollment.

Continuing Students (Old Students)

2. Student uploads the Form 138 (Card) in jpg or pdf form.
3. Enrollment staff notifies the applicant if his/her application is accepted.
4. Applicant pays the application and registration fee and tuition through the electronic
payment system by going to the Online Payment System at CEIS website and click the “pay now” button and follow the procedures.
5. Enrollment staff sends a copy of the certificate of registration to the student through email within 48 hours.
6. Student prints the certificate of registration as proof of enrollment.

OPENING OF CLASSES for SY 2022-2023
• Opening of classes for SY 2022-2023 will be in August, 2022 unless there are government announcements that will prevent us from starting the classes. In case there will be changes, you will be notified through posters in the CEIS FB page or through email.
