This application form is for use by parents to apply for DS Achieve's Resources into Homes service, focusing on Speech and Language.
Once DS Achieve has received your application, you will receive an email to confirm it is being processed and a likely timeframe in which your Resources into Homes training session will take place. The timeframe will depend on the level of demand at that time.
**You must purchase the required resource ready for the training, unless you are in receipt of certain benefits in which case DS Achieve will purchase the resource for you.**
SECTION 1 - must be completed by everyone
SECTION 2 - only complete this section if you are in receipt of income related benefits (i.e. Universal Credit) or Personal Independence Payments (PIP) for yourself. If your application is successful, you will be required to provide written proof of your receipt of the relevant benefit.
