Use this form to nominate a library in Herkimer, Madison, Oneida, or Onondaga County for CLRC's Library of the Year Award. Nominations are open to any academic, public, school, or special library (including archives and museums) across the four counties.

These nominations must be self-submitted with only ONE per institution. Any nominations received after the first has been submitted will be disregarded.
