Please read the questions closely and answer exactly as asked. This information is important for sacramental records and must be entered as asked. Requested letters and certificates must be provided for records.
The Church requires us to collect some basic information in advance of receiving Sacraments. Please complete this form no later than September 30, 2022.
The parish requires a copy of every student's baptismal certificate to attach to this registration form, even if you have provided it before for something else. If you have misplaced your copy of the certificate you can call the child's church of baptism and request that the parish mail, email or fax a copy of the baptismal certificate to St. Paul. If your child was baptized at St. Paul, and you have lost the baptismal certificate, you can contact PAM BRADY at the parish office to get the verification that you need: 330-499-2201.
If the Candidate's Sponsor is NOT from Saint Paul Parish, the Sponsor must submit a letter from their Parish of Registration verifying eligibility to be a Confirmation Sponsor.
Canon Law:
Sponsors must have the following qualifications:
A sponsor must be at least 16 years old.
A sponsor must be a fully initiated Catholic (one who has celebrated Baptism, Confirmation, and Eucharist) who “leads a life in harmony with the faith and the role to be undertaken.”
If married, the marriage must be valid in the Catholic Church
A sponsor must not be bound by any Church penalty.
A sponsor must not be the parent of the one to be baptized/confirmed.

The Sacrament of Confirmation will take place: November 5th, 2022 at St. Paul Parish. There will be two Mass options - 10:30 AM or 2:00 PM.
If your family is not registered at St. Paul Parish your student should receive their sacrament at the parish which you are registered. There are some circumstances when a pastor will give permission for students to receive sacraments at other parishes; you must ask permission from your pastor if you wish for your student to receive their sacrament at St. Paul.
