14+yr old Teen Girl's Volunteer Form
GET EXCITED! Registration for July 2024 Supergirl Summer Camp is Officially Open!

Do you have a teen girl between the ages of 14-18? We would love to have her join us the week of July 8th-July 12th. 

Each day of the camp is going to have different themes relating to possible career paths that may inspire her to dream big. We will also have all sorts of games, crafts, and activities throughout each day that are going to be fun for all ages! 

Our goals for her will be to help her gain self-confidence and for her to learn her natural leadership ability. Our teen girls will serve as our volunteers in various leadership and responsibility roles, therefore their registration fee will be waived. 

We will need help in various roles, such as:
  • Small Group Leaders (always working alongside an adult leader)**
  • Game/Activity Coordinators
  • Social Media Managers
  • Art/Craft tables set-up & tear down
  • Audio/Technical Support for our Guest Speakers
All our teen volunteers will earn credit hours they can apply towards other organizations and commitments*

Should she choose to be a small group leader, she will have the pleasure of working with a small group of younger girls ranging in ages 7 to 13. She will be working alongside a female adult leader to guide the group, working together as a team.

We will also be working to provide additional classes/trainings in courses such as CPR/First Aid (outside the week of the camp), which look great on resumes for those who may want to earn extra income as babysitters, etc. while in school.

Our teen girls will build a lasting relationship with the younger girls she mentors and assists. She will help the girls navigate through the day's activities, crafts, games, and projects. And we promise she is going to have a blast too!

We want to show our continued support to our teen volunteer's dreams & aspirations after they graduate, and therefore through volunteering with us, they may qualify for a scholarship as a graduating high school senior **.

*Supergirl Summer Camp is a 501c.3 non-profit organization.  Female teens who are needing volunteer hours, community service hours will receive 1 hour awarded for each hour volunteered during the course of the week. 45hrs Total per camp.

**Female teens who seeking to qualify for the senior scholarship will be required to serve as volunteers for a minimum of 90hrs.

In consideration of being allowed to participate on behalf of the Supergirl Summer Camp, Inc 501c.3 program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

Participation includes possible exposure to and illness from infectious diseases including but not limited to  COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child's participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during her presence or participation, I will remove her from participation and bring such to the attention of the nearest official immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Supergirl Summer Camp, Inc. their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY APPROVING IT, AND APPROVE IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
