-Fill out this form for yourself AND each of your participants. Please fill out a separate form for each individual, including yourself. Make sure your Panel Title is spelled and formatted the exact same for each form, as that is how we will categorize responses.
-Indicate N/A in a box that does not apply to you, rather than leave it blank.
-Please provide each participant's professional name and for schools that have multiple campuses, please indicate the correct campus.
-Please provide correct and complete information in order to help us avoid errors in the printed program.

DEADLINES:

-All session abstracts are due by September 15th and can be e-mailed to the MMLA directly at MMLA@luc.edu.
-Due to printing deadlines, participants who have not paid the required membership and registration fees by July 1st will risk being removed from the convention program.

Audio-Visual Equipment Requests
The MMLA will provide equipment within reason. Because rental agreements for audio-visual equipment must be signed months in advance, and any late requests for audio-visual equipment incur hefty fees, the cost of last-minute requests will be the responsibility of the session organizer or chair. Prices are approximate and may be subject to change without notice. To avoid these costs, please submit this form by June 1, 2021. All participants who require a laptop during their presentation must bring their own laptop. The audio-visual vendor will provide all of the necessary cables.
