Please fill out this form if you intent to sign up for Hockey camp with Coach Tian this summer in Ontario  Center Ice, (201 S Plum Ave, Ontario, CA 91761). We will have a 5 day camp in June - Sunday 6/16/2019 through Thursday 6/20/2019. The camp price will be in the range of $400-$450 per skater, depending on how many skaters attend. Once we have all skaters information, we will announce the final price. To complete the registration we will ask you to pay a deposit of $400 so that we can reserve the ice. We will ask you to pay the rest once we know how many skaters will attend.

Camp details:
 
1.    From 8:00 AM – 3:00 PM includes 3 hrs on ice and 3 hrs off-ice training. Lunch will be provided.
2.    Ontario Center Ice Arena address: 201 S Plum Ave, Ontario, CA 91761
3.    Check-in will start at 7:00am on June 16th. Please arrive by 8:00 am every morning to ensure a prompt start.
4.    Full gear is required for on-ice practice.
5.    Please bring an extra hockey stick for dryland training purpose (for use with iron puck).
6.    Please wear proper shorts and shoes for dryland training.
7.    Make sure to bring water bottle for your child. Pack healthy lunch and snack for your child if you don't want them to eat provided lunch.
