THIS FORM SHOULD BE FILLED IN, INDIVIDUALLY BY THE GUARDIAN(S) OF A PHS MARCHING BAND & COLOR GUARD STUDENT! (It is necessary to complete the form each school year as previous year's data is archived for privacy purposes).
This form is required for all students who may be involved with the Prospect HS Marching Band & Color Guard and Winter Color Guard for this school year. In the event we need to seek medical care for your student, please provide complete and accurate information. All information will remain confidential and will only be shared with care providers as needed for the health and safety of individual students. The last section is an additional media release form (required by school) as well as volunteering opportunities for the group.
