PLEASE COMPLETE THIS REGISTRATION BY MARCH 26, 2023.

Please fill out all portions of this form and hit submit. (One form per youth attending Moroni's Quest.)

IMPORTANT: If you do not attend Raymond Junior or Senior High School, or do not currently live in Raymond, you will need to seek approval before registering for Moroni's Quest. Please call Carolyn Palmer at 403-382-7510

TO COMPLETE THE REGISTRATION, PLEASE ALSO DO THE FOLLOWING:

1) Send payment of $40 per youth (maximum $80 per family) to your ward to complete the registration. You will need to pay through a tithing slip by ticking the box "OTHER" and writing Moroni's Quest.

2) Download the Church's PDF Permission and Medical Release Form and return to your Ward's YW president. (More information under Section 3).
