Per district Transportation Guidelines:  Only students that are zoned to Caleb's Creek may apply for transportation to and from school.   Complete only if there is a need for a bus stop.  Daycare stops will not be assigned unless you are residential to Caleb's Creek. Students will not be able to ride home with a friend as we have a limited number of seats available per bus route.  There will also not be any assignments made this year to an alternate stop.  There will not be any alternate stops or requests made at this time to a second stop or relative, etc.  The stop is only created to and from the home address that is assigned as residential to our school.  Only fill out if you need transportation- one per student and if you are in zone for Caleb's Creek.  Check just one box below.   If you check need am/pm you do not need to check it again that is for children who only need one am or pm.
