This is a voluntary, anonymous form where we are asking you to let us know about contributions you have donated during the month of December to local charities.  You may fill out the form as many times as you like until Dec. 22.  Please do NOT provide your name in your response.

HPCL will report a general summary on Dec. 23rd (the 1st day of HPCL) showing the generosity of our community to support others.
