I understand that these classes are intended for highly motivated students who seek quality instruction and experience in the performing arts, and  I will attend regularly and concentrate on the activities at hand. Sessions are non-refundable.

I, ____________________________ give TAPED permission to use my or my child's photograph or photographic image, video in official TAPED business, including: website, newsletters, social media, marketing, etc.  I understand that photographic images or video may be used for news organizations and promotional purposes.  

I hereby waive any right that I may have to inspect or approve the finished product in which a photographic or video image may be used including the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied.

By typing your name you agree that it will serve as your signature. By signing below, I agree to abide by TAPED's terms and conditions as mentioned in the coaching agreement, release of liability and photo release forms.

Classes are 2hrs long and will sometimes run longer.


Books:
If required, The student is responsible for the cost of books. Upon request, the teacher may purchase books for the student and be reimbursed along with the student’s next payment.


Lesson Cancellation:
As I, too, am a working artist, I will also give 24hr notice for sessions needing to be rescheduled due to my
conflicts. Class fees and will not be refunded for lack of teen's attendance.

Make­up Lessons:
If the Coach is unable to give a scheduled lesson, a make­up lesson will be scheduled within one week.

Tardiness:
Lessons will start on their scheduled time. If a student is tardy, the time will not be made up.
Unless we are notified in advance, Tardiness in excess of 30 minutes will result in non admittance to the online session. The lesson fees will not be refunded, and there will be no make­up lesson scheduled.

Lesson Requirements:
Students should bring the following items to every lesson: Notebook, a pencil and sides.
Students should come to each lesson with all of the week’s assignments completed and be ready to discuss what they are working on.

Code of Conduct:
Students are to refrain from using inappropriate or disrespectful language and/or behavior during lessons. The first offense will result in a verbal warning.
The second offense will result in a written warning, and a conference may be required.
The third offense will result in automatic withdrawal from lessons with Paige Henry/Taped Academy.

Withdrawal From Lessons:
Each Agreement states the length of time you agree to take classes for.
