PARENTS/GUARDIANS ACKNOWLEDGMENT

Please be advised that, due to the COVID-19 pandemic, the School District of the Chathams Board of Education will not be providing students/athletes with district transportation to and from School-sponsored events or activities. Therefore, in order for a student to participate in an extracurricular and/or athletic activity that occurs away from school premises or property, parents/guardians must arrange for transportation services for their child to and from the activity.

I hereby release, waive, discharge, hold harmless and covenant not to sue the School District of the Chathams Board of Education, its individual members, officers, agents, servants, or employees (hereinafter referred to as the “Board”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my minor child (identified above), myself, any third party and/or any property damage to my child, myself and/or any third party as result of, or in any way arising out of my child traveling to and or from School-sponsored activities in a vehicle not owned or operated by the Board. *
