This is for children in the first phase return only.  If your child is currently in Reception, Year 1 or Year 6 please fill in.  Fill in one questionnaire for each eligible child.  


The government have issued guidance on schools opening after Spring Bank.  They have indicated classes will contain 15 children which will operate in a bubble with the same staff each day.  

We need to know the following information so we can plan the  correct staffing, equipment and classroom space.

Arrangements for current key worker children will remain.
